Unsecured prescription medication in a kitchen cabinet and cleaning supplies under the sink violated the central medication storage and hazardous substance storage requirements. Postural supports and partial bed rails were in use without physician orders, contravening postural support regulations. Additionally, surveillance cameras in two bedrooms infringed residents’ personal privacy rights, and both exit gates in the backyard were locked, violating fire and panic protection regulations. The LPA required the removal of cameras, secure medication storage, removal of unauthorized rails, and unlocking of exit gates, with all corrections due by 05/17/2022.
Across both visits, a pattern emerges of generally adequate day‑to‑day operations but recurrent lapses in the secure storage of hazardous items and adherence to resident safety protocols. While the facility responded quickly to the 2021 deficiencies, the 2022 inspection revealed similar risks that were not fully mitigated. The repeated requirement for corrective action underscores a need for more robust internal checks to ensure that toxins, medication, and other dangerous items remain inaccessible to residents, and that personal privacy and fire safety regulations are consistently upheld. The licensing authorities have documented these deficiencies and have provided clear plans of correction, emphasizing the importance of timely compliance to avoid civil penalties.
