The first visit on 21 July 2022, led by Licensing Program Analyst Joe Katrdzhyan, responded to a complaint alleging inadequate staff supervision, infrequent emptying of bedroom waste baskets, malodorous rooms, and substandard meals. Interviews with the administrator, staff, and residents, along with a walk‑through of randomly selected rooms and the kitchen, revealed no evidence supporting these allegations. Staff schedules were documented as sufficient for the shifts, waste baskets were observed empty and rooms clean, and the meal program met dietitian‑approved standards with diverse menus and the availability of second servings. The report therefore concluded the allegations were unsubstantiated.
The second investigation, conducted on 14 November 2022 by Licensing Program Analyst Bennette Pena, addressed a claim that the facility unlawfully evicted a resident (Resident #1) after a hospital stay. Review of the resident’s medical chart, re‑assessment notes, and staff interviews established that the Regional Nurse had determined the resident’s medical condition had escalated to a level of care that the facility could not provide. However, the facility failed to follow the required eviction procedure, including issuing a thirty‑day written notice to the resident’s responsible party and notifying CCLD. The investigation found these procedural omissions, meeting the preponderance of evidence standard, and therefore substantiated the allegation of unlawful eviction. The report cites CCR 87224(a)(4) and includes a plan of correction with a due date of 21 November 2022.
Both investigations highlight distinct compliance issues at Santa Anita. The July findings suggest that, while staff concerns were raised, the facility’s operating procedures and environmental conditions were within regulatory expectations.
