On July 2 2024, an unannounced pre‑licensing evaluation of IVY PARK AT PALOS VERDES found the facility fully compliant with all state requirements. Inspectors Wendy Gibbs and Perry Scott met with Administrator Kelley Koul and documented that all resident rooms, bathrooms, common areas, kitchens, and walkways were clean, well lit, and free of hazards. The five‑story building—housing 37 Assisted Living units and 61 Memory Care units—met Title 22 standards, and safety systems including smoke, carbon‑monoxide detectors, fire doors, and fire extinguishers were operational. No deficiencies were noted, and the facility’s Emergency and Disaster Plan, liability insurance, and medication storage protocols were deemed adequate. The report was forwarded to the Central Applications Bureau with an exit interview conducted with Kelley Koul.
On September 13 2024, a second unannounced visit addressed two Special Incident Reports of resident falls. Inspector Wendy Gibbs met with Regional Operations Specialist Krystal Jenkins and reviewed documentation for Resident R1 (distal radius fracture) and Resident R2 (subtrochanteric fracture requiring surgery). Walkways, common rooms, and the residents’ rooms were again observed to be clean, well lit, and free of obstructions. Although R1 had a minimal fall history and lacked a formal fall plan, R2 was identified as a fall risk due to medical diagnosis. No health or safety violations were found during the tour, and the facility continued to maintain proper staffing and safety equipment.
In a May 1 2025 visit, the same inspector conducted a case‑management review following a Special Incident Report of a witnessed fall on April 12 and a subsequent Death Report for Resident R1 on April 24. Administrator Joe Saldana provided access and discussed the incidents. As with previous inspections, walkways, common spaces, and resident rooms were inspected and found compliant, with no safety violations reported. Across all three visits, IVY PARK AT PALOS VERDES maintained consistent adherence to regulatory standards while confronting ongoing resident‑fall incidents; no corrective actions were required, and the facility’s operational protocols remained in good standing.
