Almond Grove Assisted Living Facility (facility number 345 002 854) underwent four unannounced investigations by the California Department of Social Services, Community Care Licensing in 2023. All visits were conducted by Evaluator Talwinder Bains and involved record reviews, facility tours, and interviews with staff and residents. The first investigation, prompted by a complaint that staff had tried to hit a resident, concluded that no evidence supported the allegation; the allegation was marked “Unfounded” and no citations were issued.

A second investigation, triggered by a complaint about the facility’s failure to enforce COVID‑19 isolation protocols, found that the facility did not relocate either of two residents when one tested positive in December 2022. The department cited a deficiency under the Infection Control Requirements (CCR 87470(b)(3)), issued a plan of correction with a May 3 2023 due date, and documented the finding as “Substantiated.” The third investigation addressed a complaint that staff had not provided accurate information to a resident’s responsible party regarding COVID‑19 status. Records and interviews revealed that the facility had communicated information promptly and accurately, leading to an “Unsubstantiated” determination. The fourth investigation focused on a complaint that staff did not provide proper assistance to a resident in care; the investigation likewise found no evidence to support the allegation and ruled it “Unsubstantiated.”

Across the four visits, a pattern emerged: while staff and residents reported that care was generally respectful and attentive, the facility’s infection‑control practices fell short of state and public‑health guidelines. The substantiated deficiency highlights a serious compliance gap that could pose health risks to residents. The department’s requirement for a plan of correction underscores the need for the facility to promptly review and adjust its COVID‑19 protocols. The unfounded and unsubstantiated findings indicate that the other complaints lacked sufficient evidence, but the repeated scrutiny suggests that the facility is under continued regulatory observation and must demonstrate sustained improvements in both resident safety and administrative accuracy.
