Crestavilla, a 250‑bed capacity residential care facility for the elderly located at 30111 Niguel Rd, Laguna Niguel, underwent a series of regulatory reviews that consistently showed compliance with California’s Title 22 requirements. In September 2022 the administrator, Cynthia Williams, participated in a COMP II interview that confirmed her understanding of licensing, admission policies, staffing, emergency preparedness, and reporting. A subsequent pre‑licensing visit in December 2022, conducted by Licensing Program Analyst Joseph Alejandre, verified the facility’s physical plant, fire clearance, signal system, medical and toxic‑chemical storage, and food service supplies. No deficiencies were identified, and the analysts confirmed that the facility was ready to receive final approval and that a post‑licensing inspection would occur within 90 days of licensure.
In early 2023, following the death of Resident #1, two unannounced health‑and‑safety inspections were conducted by LPAs Sean Haddad and Dwayne Mason Jr. on February 24 and April 13, respectively. Each visit included a review of the resident’s room, confirmation of adequate food supplies, operational utilities, and emergency equipment. The investigators found no health or safety violations and determined that the death was due to natural causes, as documented in the death certificate. The administrators provided all requested documentation, and no Title 22 deficiencies were cited.
