Inspection Report Summary for Hollywood Hills Senior Living

Pacifica Senior Living Hollywood Hills, a 740‑type facility with a capacity of 120 residents, was inspected on 24 June 2021 for its annual required visit. The licensing program analyst observed a comprehensive infection‑control program that included universal screening at a single entry point, temperature checks, hand‑sanitizing stations, and clearly posted safety signage. The facility had a current emergency disaster plan, a COVID‑19 mitigation plan approved by CCLD, and maintained a fully vaccinated staff roster with weekly testing for a rotating 25 % sample. An isolation wing with dedicated entrances and staff monitoring was in place for symptomatic or positive residents, and no deficiencies were cited during this inspection.

Between November and December 2021 the agency conducted a series of unannounced case‑management visits to investigate several self‑reported resident‑to‑resident and resident‑to‑staff incidents. On 15 November a resident alleged a sexual encounter with a young man, while on 11 September a resident was found near an ice‑cream shop and later reported physical assault by an unidentified individual. On 24 December a resident claimed to have been assaulted by women who allegedly threw him against a door. In each case the analyst reviewed documentation, interviewed staff and residents, and found no immediate health‑and‑safety concerns or observable deficiencies. However, due to conflicting testimonies, the need for further investigation was noted in every report.

A follow‑up visit on 10 May 2022 addressed an incident reported on 4 May involving alleged inappropriate touching between two memory‑care residents. Interviews revealed that the alleged victim could not recall the time or date, a witness had no clear memory, and staff had no evidence of the event. Again, no health or safety issues were observed. Throughout these investigations the facility’s infection‑control compliance remained solid, but the recurrence of multiple resident‑to‑resident and resident‑to‑staff misconduct allegations indicates an ongoing risk to resident safety that may warrant continued oversight and additional preventive measures.
