The 08/20/2021 office visit was a COMP II telephone check that confirmed the administrator’s knowledge of licensing duties, staff qualifications, and program policies. The 09/23/2021 pre‑licensing inspection, an unannounced visit, confirmed that the transition of ownership from Facility #197609905 to #197610184 had been completed, that residents were present and well‑maintained, and that fire and safety systems met state standards.
