Inspection Report Summary for Wildomar Senior Assisted Living

WILDOMAR Senior Assisted Living (Facility Number 335530171) underwent a series of licensing visits between February 2024 and April 2025 as part of its transition to new ownership. In early February, administrator Cristina Miller completed a COMP II interview confirming her understanding of the California Code Title 22 regulations, signed the required LIC 809, and demonstrated compliance with operational, staffing, and emergency preparedness requirements. The same procedure was repeated in early April with the new administrator, Rose Yousefian, establishing continuity of regulatory knowledge during the change of ownership.

A pre‑licensing inspection on April 24, 2024 revealed that while the physical plant, safety systems, and resident accommodations were in good repair, the facility had not yet completed essential documentation—an updated Plan of Operation, admission agreement, and facility sketch. Moreover, the on‑site in‑ground pool, though gated and locked, was promoted as an amenity and listed in activity materials despite not being part of the official facility plan. Because of these omissions, the inspection was deemed incomplete and a second visit was scheduled pending corrective action.

By October 4, 2024, a subsequent pre‑licensing visit by LPAs Paola Guerrero and Beena Singh confirmed that the missing documentation had been addressed and all regulatory requirements were met. The inspection concluded without corrections, and a Comp III presentation was completed. The facility’s infrastructure—fire suppression, smoke and carbon monoxide detectors, medication storage, and resident records—remained compliant throughout this period.

An unannounced case‑management deficiency visit on April 4, 2025 highlighted a separate issue: the administrator was unable to locate the admission agreement for Resident #1, a vital record for demonstrating admission compliance. This deficiency, identified as a potential risk to resident rights, will be formally issued and requires prompt rectification. Across the series of visits, WILDOMAR maintained robust physical safety and resident care standards, while documentation and administrative record‑keeping have been the primary compliance challenges. The facility’s ability to correct earlier deficiencies suggests a responsive management team, though ongoing vigilance is necessary to ensure all resident records and operational documents remain current.
