The June 2022 complaint investigation addressed a claim that the home was not allowing residents to choose their own hospice agency. After interviewing residents, reviewing resident files, and speaking with the administrator, the LPA determined the allegation was unfounded, citing that residents were indeed free to select a hospice provider and that no records supported the allegation of forced change. The complaint was dismissed and the report was made available for public review.

The November 2022 evaluation identified a single deficiency: two residents were observed with half bed rails that lacked a physician’s written order, a violation of CCR 87608(a)(3). The administrator was required to obtain the necessary documentation or remove the rails by the end of the month. In addition, the LPA noted a lapse in visitor screening and mask use by a caregiver and hospice nurse, though this did not result in a formal deficiency. The December 2023 visit, conducted with five residents, repeated the focus on infection control and medication review; the report did not highlight new deficiencies, suggesting the facility remained in compliance with most licensing requirements. Overall, the pattern indicates a generally well‑maintained home that occasionally needs to reinforce postural‑support documentation and visitor‑screening protocols.
