The February 2021 case‑management visits were prompted by a resident’s allegation that a staff member had struck her while she was in bed. Administrator Erin Mahoney coordinated interviews with the resident, staff, and the resident’s caregiver. A body check revealed no bruising, and a medical evaluation by the resident’s primary‑care physician concluded that the resident was confused and disoriented, with no evidence of physical abuse. Consequently, no further action was taken and the incident was closed.
A subsequent visit in November 2021 addressed a separate allegation that a staff member had behaved inappropriately in a resident’s room. The staff member was immediately removed from the facility, and the administrator notified law enforcement, Adult Protective Services, the Long‑Term Care Ombudsman, and the resident’s family. The licensing analyst conducted interviews and requested staff and resident files, but the investigation was not completed during that visit; a follow‑up was scheduled to gather additional information.
The required annual inspection in January 2022 focused largely on infection‑control practices, which were found to be adequate. However, the evaluator noted several compliance gaps. The facility lacked current physician reports for all residents with major neurocognitive disorder and did not have completed hospice care plans for any of the hospice residents. Several residents were housed in rooms without appropriate fire‑clearance for bedridden status, and bed rails were used without proper orders. Staff records revealed that many employees had not completed required annual training, including first aid and CPR, and newly hired staff lacked documented training verification. The evaluator also observed that the facility’s COVID‑19 incident reporting had been submitted to an incorrect fax number. While the facility’s physical plant, including kitchens, dining areas, and common spaces, met safety standards, the administrative documentation and staff training deficiencies highlight ongoing risks to resident safety and regulatory compliance.
