The Administrator committed to training staff for at least one hour and to submitting proof of training by the set deadline. Aside from this deficiency, all other areas—including safety equipment, linens, and resident files—met regulatory standards, and the evaluator issued a formal plan of correction.
Finally, a required one‑year unannounced inspection in December 2024 confirmed that the facility continued to operate in compliance with the 740 licensing requirements. The evaluator met with both Mary Grace Suba and Cle o Brelin, reviewed current resident records, and found no new violations. The facility’s consistent adherence to health, safety, and medication protocols, coupled with timely corrective action when a deficiency arose, demonstrates a stable and compliant operation throughout the reporting period.
