CONEJO VALLEY HOME CARE, a 740‑type residential facility with a capacity of six residents, underwent a series of licensing evaluations. The initial pre‑licensing inspection on February 3 2023 confirmed that the property met Title 22 fire safety, kitchen, bathroom, and common‑area requirements. All hard‑wired smoke alarms and carbon monoxide detectors functioned, the fire extinguisher was new, and the kitchen, bedding, and outdoor areas were clean, adequately stocked, and secure. The facility was provisionally licensed pending the completion of the required paperwork and staff training.
One year later, an unannounced inspection on February 21 2024 revealed a different picture. While the physical plant remained compliant—fire safety devices were operational, the kitchen was well‑stocked, and the bathrooms met accessibility standards—the audit uncovered significant documentation gaps. Four of six resident files lacked the required Identification and Emergency Contact form (LIC 601), five lacked the Resident Appraisal form (LIC 603A), and all six files were missing the Health Screening Form (LIC 503). Personnel records also fell short: all staff were missing the Health Screening Form and one lacked the Personnel/Employment form (LIC 501). Moreover, a critical regulatory deficiency was identified in night supervision: both night staff had not completed the mandated first‑aid/CPR training, posing a safety risk to residents during the 10 p.m.–6 a.m. shift.
Despite these record‑keeping shortfalls, no medication errors or infection‑control lapses were found. The facility’s central medication storage was locked, labeled, and expiring dates were verified. The LPA noted that the facility maintained adequate personal protective equipment and a clear cleaning protocol. The deficiency in night‑staff training was addressed through a plan of correction: the facility’s representative committed to providing CPR and first‑aid certification to the night staff by March 1 2024, with evidence of completion submitted to the department. The LPA’s findings underscore a pattern of strong operational and safety practices, tempered by lapses in documentation and staff certification that the facility has already pledged to correct.
