The facility subsequently completed an in‑service training on medication documentation and provided documentation to the licensing agency, satisfying the required corrective action.

A second unannounced investigation conducted on April 12 2022 addressed allegations that residents were not receiving food on time, were served cold meals, that staff failed to follow illness‑prevention protocols, and that the facility harbored bed bugs. After reviewing meal schedules, interviewing staff and residents, and inspecting the kitchen, the evaluator found no evidence to support any of these claims. Consequently, all four allegations were determined to be unsubstantiated.

While staff and administrators consistently reported adherence to hygiene, meal service, and illness‑prevention standards, the licensing agency could not establish a preponderance of evidence for the remaining complaints.
