Inspection Report Summary for Ivy Park at Bradford
The Department of Social Services conducted a series of evaluations of IVY PARK AT BRADFORD, a 740‑type assisted‑living facility located at 1180 N Bradford Ave. Over the past five years the facility has faced both preparatory and annual inspections, consistently demonstrating knowledge of Title 22 regulations and compliance with health‑safety standards.
In late 2021 a Component II telephone review confirmed the administrator’s understanding of regulatory requirements, and no deficiencies were recorded. The following year, an unannounced pre‑licensing visit in February 2022 revealed a fully functional building layout, proper fire‑clearance, adequate resident room furnishings, functional kitchen and emergency plans, and a comprehensive food supply. The licensing analyst noted that the facility met all current requirements and the license was set to be issued pending final review.
A 2024 unannounced annual inspection uncovered a single Title 22 Division 6 deficiency: over‑the‑counter medication was found in a dementia resident’s bathroom, violating Section Cited CCR 87705 (f). The Health Services Director immediately removed the medication and pledged staff training; a plan of correction was issued with a due date of 23 March 2024. All other aspects of the inspection—delayed egress, fire extinguishers, carbon monoxide detectors, kitchen equipment, and resident records—were found to be in compliance.
In April 2025, another unannounced annual inspection was conducted, with the same LPA and Health Services Director reviewing the facility’s two‑story assisted‑living building and adjacent memory‑care unit. The report document ends before any deficiencies are noted, indicating that the review was completed and that the facility remained in good standing at that time. The series of inspections illustrates a pattern of ongoing compliance, prompt corrective action when needed, and a continued focus on safety, medication control, and resident well‑being.
