Over the past two years the California Department of Social Services, Community Care Licensing Division (CCLD), has conducted several unannounced visits, most of which confirmed that the facility remains in compliance with Title 22 regulations and its own approved mitigation plans.
In the first visit on 06/30/2021, the Licensing Program Analyst (LPA) noted that Land of Peace 2 had an up‑to‑date mitigation plan and adequate COVID‑19 screening, hand‑washing supplies, personal protective equipment, and signage. No deficiencies were cited. Four months later, on 04/21/2022, a case‑management visit revealed a serious incident: Resident #1 (R1) had been found unsupervised outside the facility at 2:55 a.m. on 04/17/2022 after a power outage on 04/15 had disabled one of the five auditory alarm devices that monitors exits for residents with dementia. The LPA confirmed that all alarms functioned correctly at the time of inspection, and R1 was safely recovered. The agency issued a deficiency (Section 87705) for the previously non‑functional auditory device, requiring correction by 05/20/2022 and warning of possible civil penalties if uncorrected.
Subsequent unannounced annual inspections on 05/03/2022 and 11/07/2023 found the facility in full compliance. Inspectors praised the cleanliness and safety of the building, the presence of appropriate signage for oxygen use, the adequacy of fire and evacuation plans, the proper functioning of smoke and carbon‑monoxide detectors, and the availability of fresh linens, hygiene supplies, and therapeutic activities.
