Inspection Report Summary for Maple House

Maple House, a 740‑type residential facility located at 410 Pennsylvania Avenue in Santa Cruz, underwent two formal licensing evaluations by the California Department of Social Services (CCLD). The first, an unannounced pre‑licensing inspection on August 7 2024, was conducted by Licensing Program Analyst Steve Chang and met with Administrator Ronnie Lising. The second, a required one‑year unannounced inspection on August 27 2025, was performed by Analyst Marcella Tarin and met with Administrator Rose Anne Roxas. In both visits, the evaluator toured the interior and exterior of the property, including all resident rooms, dining areas, kitchens, bathrooms, storage areas, and outdoor spaces.

During the pre‑licensing visit, the evaluator observed 4 staff members and 13 residents, confirming that personal‑rights posters, an Administrator Certificate, and a functioning call‑button system were present. All safety equipment—fire alarms, smoke and carbon‑monoxide detectors, fire extinguishers, emergency lights, and door alarms—was inspected and found operational. Food supplies met regulatory standards with at least two days of perishable items and seven days of non‑perishables; refrigerator temperatures ranged from 37 °F to 40 °F and the freezer held at 0 °F. Resident rooms were adequately equipped with beds, lighting, and storage, while bathrooms met hygiene and safety requirements. No deficiencies were cited, and the report was signed by the Administrator and the analyst.

The annual inspection mirrored these findings. Four staff and 11 residents were present; all storage areas for medications, knives, and cleaning products remained locked and inaccessible to residents. Fire safety continued to meet standards, with a recent third‑party inspection on April 4 2025 and the latest extinguisher service on March 11 2025. Water temperatures in resident bathrooms ranged from 105 °F to 110.8 °F, and the evaluator confirmed that all safety devices functioned properly. Again, no deficiencies were recorded.

Across both visits, Maple House demonstrated consistent compliance with California’s licensing requirements. The repeated absence of deficiencies, coupled with regular verification of food safety, temperature controls, and safety equipment, indicates a stable and well‑maintained environment for residents. The facility’s ongoing record‑keeping and prompt communication with CCLD further underscore its commitment to high‑standard care and regulatory adherence.
