In May 2022, Analyst Sarena Keosavang returned for a case‑management visit following two theft incidents reported on 24 March 2022: a resident’s wedding ring and chain went missing, and another resident was the subject of fraudulent bank activity. The facility’s internal investigation led to a police report (No. 2022‑15099). During the inspection the agency recorded no additional deficiencies, indicating that the incident had been appropriately addressed.

A July 2022 unannounced inspection highlighted infection‑control lapses.
