The California Department of Social Services investigated PASADENA HIGHLANDS, a 740‑type assisted‑living facility with 245 beds, on several occasions in 2021 and 2022. Two 2021 complaints were found to be substantiated. The first, received on 04/22/2021, alleged that residents were not receiving timely assistance because of staff shortages. An unannounced visit on 05/03/2021 confirmed that the assisted‑living floor, which normally had four to five caregivers, was operating with only one or two, causing residents to wait up to an hour for showering, dressing, or escorting to meals. The evaluator cited CCR 87411(a) and required a plan of correction by 05/17/2021. The second complaint, filed on 06/07/2021, concerned a resident’s elopement from the memory‑care unit due to inadequate supervision. The 06/09/2021 inspection revealed that a delayed‑egress exit door was not functioning correctly and that a resident with dementia had slipped out of the unit, was found 1½ miles away the same day, and was unharmed. Again, the evaluator cited CCR 87411(a) and required corrective action, including staff training and an updated care plan, by 06/11/2021.

In early 2022 the agency addressed additional allegations that staff were not following care plans, failing to assist with incontinence, and allowing rooms to become malodorous. The unannounced visits on 02/02/2022 and an earlier visit on 02/02/2022 (details truncated) found the complaints unfounded; no evidence of noncompliance was identified. These findings suggest that while staffing shortages and supervision lapses remained significant issues in 2021, routine operational practices such as care‑plan adherence and environmental hygiene did not trigger regulatory deficiencies at that time. The repeated focus on personnel requirements underscores the importance of maintaining sufficient, competent staffing and robust supervision protocols in community care settings.
