Each visit documented that the facility maintained appropriate environmental conditions, resident records, infection control protocols, and staffing arrangements. The January 2024 audit also verified that the resident census matched the approved capacity, that four of six residents were ambulatory, and that essential supplies—including personal protective equipment and perishable food—were adequately stocked.
In March 2024, a case‑management incident visit was prompted by a self‑submitted LIC 624 Incident Report and a LIC 624A Death Report concerning Resident #1 (R1). Staff had transported R1 to a local emergency department on 02 24 2024; R1 passed away in the hospital on 03 06 2024. The unannounced visit performed a welfare check on remaining residents, reviewed pertinent records, and found no safety concerns or deficiencies. No corrective actions were required.
During a December 2023 complaint investigation, Licensing Program Analyst Daniel Pena addressed an allegation that the licensee failed to issue a refund to a resident’s authorized representative.
