On 14 September 2023, a complaint that residents’ needs were unmet and that eye medications were not administered was substantiated after a review of medication administration records showed multiple missed doses of Dorzolamide/Timolol drops, Travan, Oasis Lid wipes, and Oasis Tear Plus lubricant over November 2019 through January 2020. The evaluator cited CCR 87464(f)(4) and issued a deficiency requiring proof of staff training on medication administration by 22 September 2023. The facility’s administrator, Douglas Baker, acknowledged the need for corrective action and provided a plan of correction.
A follow‑up visit on 16 January 2024, triggered by a separate complaint from June 2020, found that staff training was inadequate—no current CPR/First Aid certification was on file—and that incidents, including the theft of a resident’s ring and watch and camera equipment, were not reported to the Department. These allegations were substantiated, again under CCR 87464(f)(4), and prompted a similar deficiency notice with a due date for corrective action. The evaluator also examined the claim that staff had failed to seek emergency medical care for a resident who became unresponsive; this particular allegation was ultimately unsubstantiated after staff interviews and review of incident documentation.
Across both investigations, a pattern emerges of inadequate staff competence and oversight, deficient medication administration, and failure to document and report incidents in a timely manner. The Department’s findings underscore the potential health, safety, and personal‑rights risks posed to residents and signal the need for Villa Living to implement rigorous training, documentation, and incident‑reporting protocols to regain compliance and protect resident welfare.
