West Park Senior Living Facility (Facility Number 198603550) was the subject of three separate unannounced inspections conducted by the California Department of Social Services, Community Care Licensing Division. The first visit, on 28 November 2023, addressed a complaint that staff had mismanaged a resident’s medication. The evaluator found that the prescription order for Hydralazine dated 16 May 2023 did not match the label on the bottle dispensed in July, which had an additional half‑tablet dose that was not authorized by the physician’s order. The investigation concluded the allegation was substantiated, citing CCR 87465(e)(2) and requiring a corrective plan by 19 December 2023.

In June 2024 and January 2025, subsequent visits investigated separate resident complaints. In June, the evaluator examined allegations that staff had not followed physicians’ orders for special diets and that residents’ money had been stolen. Interviews with staff and residents, along with a review of dining room procedures and resident agreements, revealed that residents were offered appropriate choices, including sugar‑free and vegetarian options, and that no evidence of theft was found. The allegations were therefore unsubstantiated. In January, allegations centered on housekeeping deficiencies, obstructions, and a malodorous environment. A tour of the facility, observation of housekeeping activities, and staff interviews showed that the facility was clean and that housekeeping schedules were in place. Consequently, the complaint was again deemed unsubstantiated.

These inspections highlight a pattern of mixed compliance. While the facility appears to provide adequate dietary options and maintain a clean environment, the substantiated medication mismanagement incident underscores a critical safety lapse. The corrective plan due in December 2023 will determine whether procedural and documentation gaps are addressed, and ongoing monitoring will be necessary to ensure that medication administration adheres strictly to physician orders.
